Preparing for What Comes After Graduation
Starting around age 14–15, transition planning becomes a required part of every student's IEP — and it's one of the most important stages families will navigate. We help families understand transition goals, explore graduation pathways (including Regents-track diplomas, alternative credentials, and 18-21 programs), and think through vocational assessments and work-based learning opportunities.
As students get closer to leaving the school system, we help families connect early with adult service agencies — including OPWDD and ACCES-VR — so there isn't a gap in support once school-based services end. We also help families think through options for continuing education, employment, day habilitation, and community-based programs.
